Call our office to schedule a new patient appointment. Please arrive 15 minutes early to fill out paperwork and bring a photo ID and your insurance card. Be sure to check with your insurance if you have any questions regarding coverage.
Our office collects a nonrefundable Paperwork Processing Fee of $40 when you make a new patient appointment. This is applied to the patient responsibility portion of your patient office visit.
